This ordinary looking helm is quite magical it gives a +4 to armor protection. However any time a skill check requiring concentration or serious thought or is attempted this helmet will cause the wearer to suffer a horrible headache, this headache will last for 1d4 hours, during this time, all skill checks are at -5, Dexterity will be reduced by 3, all attack rolls are at -2 to hit. During the headaches spot checks are further reduced by 3

I'd limit it to only those skill checks that require concentration or serious thought. So, concentration and knowledge and disable device, yes. But swim, jump, or climb, no.

Sorry, let me rephrase. Only certain skills shoule TRIGGER it, but it would penalize all of them.

Total update to this helm.
This ordinary looking helm is quite magical. The helmet stops 25% of all critical hits. However any time a skill check is attempted this helmet will cause the wearer to suffer a horrible headache, this headache will last for 1d4 hours, during this time, all skill checks are at -5, Dexterity will be reduced by 3, all attack rolls are at -2 to hit. Characters suffering from Migranes are also at an additonal -1 to hit in bright light, loud environments (like those involving fights), fatigue. These characters are subject to a rage like that of a barbarian. The character must also make a Fort SV DC 19 or suffer from severe nausea and vomiting, the vomiting lasts for 1d6 rounds and the character will suffer 1d3 points of damage from the painful contractions and upheavel of their stomach. The helmet can only be removed after a remove curse, limited wish, wish or miracle spell is cast.

+4 to armor protection - I think this should be something like "adds +4 armour bonus" or even better "The helmet stops 25% of all critical hits" which would be akin to light fortification.
if you are saying that they suffer from nausia then use the Nausiated condition perhap modify Dazzled and Nausiated combining them as migraines can have a visual aura which impedes so much.
