Alright, I've been mulling over the idea of a town built on a graveyard for some six months now and here's what I've come up with so far.

The characters, acting separately at first, find their way to a small town built in the center of a graveyard that extends as far as the eye can see in all directions. Shortly after their arrival, strange occurances, ranging from the feeling of being watched to seeing strange apparitions, begin to plague the party. The villagers seem to be oblivious to these happenings, but the cycle of strange events continues, even grow more intense as time goes on. These adventurers came separately, but are united in the fact that they all seem to be encountering strange visions, noises, and events that seem to play on their mind. However, at night, the adventurers find that they are facing an undead horde from the surrounding necropolis.

As the adventure drags on, the hallucinations become reality, seeking to destroy the party of mishappen adventurers before they can learn of the evil the townspeople committed. Or perhaps these visions are simply trying to ward off the adventurers so that they too do not fall prey. Only time will tell, but as the clock ticks, their chances of survival decrease. And then, suddenly, it seems as if hell itself has come to earth, seeking to destroy the adventurers who choose to stay and investigate.

Also, I have not named this yet, seeing as my creative juices are limited when it comes to names. However, this is meant to be an adventure setting that can be plugged into any campaign. Just going to need some help statting it out and getting a story-line going to it.

Instead of a traditional graveyard per say, what if the town was built in some hills that were actually barrows.

Or if the graveyard did not really exist in this dimension, but rather in another dimension but in the same place. The veil between the worlds of the living and the worlds of the dead could be wearing thin in the area. That could be what was causing the hallucinations. This could lead to a lot of various situations from the veil shattering, to the characters being caught in the spirit world.

That's an interesting take, though, that would rule out what I had considered. My suggestion would be that the activities are cult related, centering around a lich who originally was part of the cult and now has caused the hauntings to scare away the characters and protect them from the rest of the townsfolk who will surely try to use them as human sacrifices.

I do, however want to keep the matter open to discussion to try to get as many ideas and takes on it as possible.

The only reasons that I made the suggestions that I did is because I cant really see any type of normal people living in a town situated in the middle of a graveyard. Outside (normal) people have to be able to make it to the town on a normal basis if for nothing else, but to resupply the town.

However this leads me to another idea.

What if the graves started to suddenly appear instead of the town being built in a gravetard. That would make more sense. Perhaps the necromancer and his cult summoned the graves. The graves could be those of the townsfolks ancestors. They could see it as a divine miracle. The normal townsfolk would be converted or sacrificed by the culties. And to go along with the necromancer theme; perhaps the cultists lost faith in their leader and imprisoned him to allow them to act as they pleased.

There are a million different scenaios and plot hooks.
