Type: Ingestion (DC 15)
Primary damage: Disorientation - 1d3 DEX damage and Dazed for 1d4 rounds
Secondary damage: Horrific images cascade through the victim's mind for 1d6 hours as if they are under a Confusion spell; 1d4 WIS damage.
Market Price: 1000gp

MindForge found the creation of ideas to be a heady experience and in using this poison granted his victims an insight into his warped and dangerous imagination. The crimson liquid has been used from time to time by alchemists, assassin, artificers and mages to bewilder their victims and weaken enemies resolve.

I like it... warped and dangerous?
Maybe warped, but dangerous? Well, maybe so on the other end of the d20 as a player.

Justin- "But I rolled a 17 against that thing's fear ability."
DM Mindforge - "That just mean's your not going comatose like Jim."
Jim- "A coma?!"
DM Mindforge: "Yeah. What do you guys thing happens, when you attack creatures born of pure fear at CR 30."
Jim - "But we are only 19th level?"
DM Mindforge - "And I warned you and warned you not to go in here yet. Even out of character I told you the other day that you shouldn't open the tomb. I gave you plenty of in-game knowledge also of the power this thing has. You guys didn't even go find the artifact that would help you defeat it, by making it corpreal again. Grenrick told you guys you would need to bring it from the other world to this in order to defeat it.?
Justin - "Can I still run?"
Jim - "Dude. grab my body."
Justin - "My load will increase and it will slow me down. I am chaotic neutral after all."
Kerry - "I'm glad I never went in the room." On my next action I am going to run back to the planar gate and go back to Grenrick's Stand. No looking back."
Jim - "great. I just lost a 22nd level character."
Kerry - "Opening the door was your idea. Justin even said let's keep going, this thing look dangerous and the DM has told us again and again not to open the door."

Jim gets mad and tears up character. "Fine. Do I have to make a new character at level one?"

"Dude. You were still alive, but I think your body was just ripped to pieces by some random act of the gods."
