This small parasite is a menace to all intelligent creatures that it comes in to contact with. It appears as a small black worm with a purple underside. It is 4 inches in length when fully grown. It favors ponds or other pools of water that are frequented by intelligent animals. They are a solitary creature and only come together to mate once per year. Each mating will yield from 1-6 live young, most of which never make it to adulthood.

The creature attacks by latching on to a victim. This can be any place, but the leeches prefer to hide in warm areas away from strong light sources. They can usually be found behind the knees, in the small of the back, in the hairline of the victim, or any other such area.

When the leech successfully latches on to a victim, they must make a fortitude save (DC12). If this is not made they begin to become ill from the natural toxins that the leech produces. They are considered “Sickened” as per the condition for 1d4 hours. After that time their body has adapted to the sickening quality of the toxins and it can begin to feed. If the fortitude save is made, the creature’s toxins will have no effect on the victim. (Note: Each creature’s toxins and chemical emissions are different. A person may be immune to one creature, but suffer from another’s attacks.)

The toxins will have certain hallucinogenic effects on the host. The toxins will cause the victim to experience lifelike illusions. The duration of these illusions is 1d6 rounds. The leech will inject these chemicals 1d3 times per day, randomly. Each time that the toxins are injected a fortitude save to avoid the effects can be made (DC12) The type of illusions that the chemicals produce will be random as well (See chart).

The effects and the illusions themselves are adjucated by the DM depending on the current circumstances the victim is in.


It lives from the emotions that it causes in a victim. The strength and severity of the illusions (and emotions) are directly proportionate to the size of the leech. For every inch in length (Over 1 inch) the DC of the effects will be one point greater. So a full grown leech (4 inches in length) will increase the DC of its effects by +3.
